Passports

Appointments are recommended when applying for a passport, please call 641-683-0045 to schedule your appointment. Passport appointments cannot be made online.

You must apply in person using Form DS-11 if at least one of the following is true: 

  • You are applying for your first U.S. passport
  • You are under age 16
  • Your previous U.S. passport was issued when you were under age 16
  • Your previous U.S. passport was lost, stolen, or damaged
  • Your previous U.S. passport was issued more than 15 years ago

If none of the above statements apply to you, you may be eligible to renew using Form DS-82, allowing you to renew by mail and not be charged the $35.00 execution fee. Feel free to contact our office to determine eligibility for renewal by mail.

Please note: If you are printing and completing your own forms, they must be printed single-sided and the margins cannot be cut off.

Adult Applicants (age 16 and older)

Passport Book

  • Form: DS-11
  • Application Fee: $130 check/money order
    Check or money order is required for the application fee. Make Payable to the "U.S. Department of State."
  • Execution (Acceptance) Fee: $35 check, cash, money order, or credit card (3% fee)
    This fee is paid separately. Payable to the "Wapello County Recorder."

Passport Card

  • Form: DS-11
  • Application Fee: $30 check/money order
    Check or money order is required for the application fee. Make Payable to the "U.S. Department of State."
  • Execution (Acceptance) Fee: $35 check, cash, money order, or credit card (3% fee)
    This fee is paid separately. Payable to the "Wapello County Recorder."

Provide U.S. Citizenship Evidence

You must submit one of the following. Your evidence must be an original or certified copy. Photocopies and notarized copies are not acceptable. 

  • Fully-valid, undamaged U.S. passport (may be expired)
  • U.S. birth certificate that meets the following requirements: 
    • Issued by the city, county, or state of birth
    • Lists your full name, date of birth, and place of birth
    • Lists your parent(s)' full names
    • Has the date filed with the registrar's office (must be within one year of birth)
    • Has the registrar's signature
    • Has the seal of the issuing authority
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Naturalization
  • Certificate of Citizenship

Present ID

You (or your parent/guardian) must present one of the following IDs to the acceptance agent. 

  • In-state, fully-valid driver's license 
  • Fully-valid, undamaged U.S. passport
  • Certificate of Naturalization or Citizenship (if the photo still looks like you)
  • Government employee photo ID (city, county, state, or federal)
  • U.S. military ID or military dependent ID
  • Valid foreign passport

Please note: 

  • Some states now issue digital ID documents (also known as mobile driver's licenses or mobile IDs). We cannot accept these digital IDs when you apply for your U.S. passport. You must continue to submit your physical photo ID and a photocopy of the ID. 
  • If you present an out-of-state ID, you must present an additional ID. 

Provide a Passport Photo

  • Each applicant must provide a photo when applying for a passport. Several specifications are required for the photo to be accepted.
  • The Wapello County Recorder's Office offers passport photo services on location. The fee is $15 for two photos. Photo services are also available at several local businesses in Wapello County.
  • The passport photograph must be 2 x 2 inches in size.
  • The image size measured from the bottom of the chin to the top of the head (including hair) should not be less than one inch or more than 1 3/8 inches.
  • Photographs must be (including, but not limited to):
    • Printed on matte or glossy quality paper
    • Clear, sharp with no printer dots/smudges, no grainy appearance
    • Centered with head facing forward with neutral facial expression
    • Have a plain white or light background
    • Taken within the past 6 months
    • Eyes must be clearly visible
    • No hats or head coverings except for religious purposes
  • Photos cannot be digitally altered.
  • Glasses must be removed for passport photos. A signed medical statement signed by a medical professional must be submitted if the glasses cannot be removed due to medical reason.
